I'm wondering how E-W could arrive at 4S when West would probably
open 1S and East would pass with 4HCP and such a flat hand.

Please explain what system might be used to force East to bid?

Besides if hearts are led, West will have to ruff two or three times and
could lose control of the trump suit.

First, I didnt make this board. The scoring is decided by how good the contract is, not if it is possible to bid it. With a strong club system, there is not so difficult to reach 4S. And with trump and diamond 3-2 there is no problem to make at least 10 tricks.
